Form Outline

The TREC OP-L form is organized into the following sections:

  1. Header and Property Identification
    Identifies the property address and links the addendum to the underlying purchase contract.
  2. Lead Warning Statement
    Provides the required federal warning about the potential presence of lead-based paint in pre-1978 residential properties and recommends professional inspection before purchase.
  3. Seller's Disclosure
    The seller must disclose their actual knowledge of lead-based paint hazards and provide any available records, reports, or inspection results related to such hazards in the property.
  4. Buyer's Rights
    The buyer either waives the right to a lead-based paint inspection or retains the right to inspect within 10 days and terminate within 14 days if hazards are found.
  5. Buyer's Acknowledgment
    Buyer confirms receipt of all seller-provided disclosure information and the EPA pamphlet on lead poisoning prevention.
  6. Brokers' Acknowledgment
    Both the listing broker and other broker confirm they have informed the seller of all federal disclosure obligations and are aware of their own responsibility to ensure compliance with the law.
  7. Certification of Accuracy and Signatures
    All parties (buyers, sellers, and both brokers) sign and date the form, certifying the accuracy of the information they provided.
