The TREC 50-0 form is organized into the following sections:

  1. Header and Property Identification
    Identifies the property address and buyer name to tie this termination notice to a specific executed contract.
  2. Basis for Termination
    Provides two checkbox options for the seller to specify why the contract is being terminated: either for failure to deliver earnest money (Option 1) or under another specific contract provision (Option 2).
  3. Note Regarding Remedies and Earnest Money
    Clarifies that sending this termination notice does not waive the seller's right to pursue other legal remedies, and that earnest money disposition is controlled by the contract terms, not this notice.
  4. Legal Advisory and Attorney Consultation
    Standard TREC advisory reminding parties to consult an attorney before signing, noting that agents cannot provide legal advice.
  5. Seller Signatures and Dates
    Signature block for the seller(s) with date fields. All sellers on the contract must sign for the notice to be valid.
