Form Outline

The TXR 2003 form is organized into the following sections:

  1. Header and Property Information
    Identifies the rental property, expected move-in date, proposed monthly rent, security deposit amount, and requested lease term in months.
  2. Applicant Identification
    Captures the applicant's legal name, all contact details, government ID information, physical description for identification purposes, and lists any co-applicants who must each submit their own separate application.
  3. Property Condition
    Records whether the applicant has physically viewed the property before applying. The form strongly encourages in-person viewing. Landlord makes no express or implied warranties about condition. Applicant may list requested repairs.
  4. Representation and Marketing
    Identifies the applicant's real estate agent representation (if any) and the marketing channel that led the applicant to the property (sign, internet, or other).
  5. Applicant Information
    Gathers the applicant's rental history (current and previous addresses with landlord contacts, move dates, and rent amounts), employment history (current and previous employers with verification contacts, income, and positions), other income sources, and emergency contact information.
  6. Occupant Information
    Identifies all additional occupants (non-co-applicants) who will reside at the property, including their name, relationship to the applicant, and date of birth. These are typically minor children or dependents.
  7. Vehicle Information
    Requires a listing of all vehicles (cars, trucks, boats, trailers, motorcycles, etc.) that will be parked at the property. Notes that state, local, and HOA ordinances may restrict parking certain vehicles.
  8. Animals
    Collects detailed information about any animals the applicant intends to keep at the property, including whether they are assistance animals. Information includes type, breed, name, color, weight, age, gender, neutered status, bite history, rabies vaccination status, and assistance animal designation.
  9. Additional Information
    A series of yes/no disclosure questions covering: waterbeds or water-filled furniture, smoking/vaping, renter's insurance, military status (including orders limiting stay to one year or less), and rental/financial history including evictions, lease breaches, bankruptcy, foreclosure, criminal convictions, sex offender status, and credit issues.
  10. Authorization
    Contains the applicant's authorization for background screening (credit, criminal, employment/rental verification), notice that the property remains on the market until a lease is fully executed, privacy policy disclosure, application fee and deposit provisions, and the applicant's acknowledgement that they have reviewed tenant selection criteria, that inaccurate information is grounds for rejection, and that statements are true and complete.
  11. Authorization to Release Information
    A standalone authorization document (page 5) where the applicant specifically authorizes current and former employers, landlords, mortgage lenders, and financial institutions to release information, and authorizes the landlord's representative to obtain consumer credit reports and background information.
  12. For Landlord's Use
    Internal tracking section where the landlord records when and how the applicant was notified of the decision (approved or not approved), and the reason for disapproval if applicable.
