Form Outline

The TXR 2016 form is organized into the following sections:

  1. Property Identification
    Links the form to a specific residential lease by property address.
  2. Tenant Information
    Records identifying and contact details for up to four tenants on the lease, including government ID numbers and personal demographics.
  3. Occupant Information
    Records identifying and contact details for up to five non-tenant occupants who have the landlord's consent to reside at the property.
  4. Named Representatives (Death Event)
    Names a representative (not a tenant or occupant) authorized to act under Paragraph 31F of the lease if all tenants/occupants over 18 die during the lease term.
  5. Animal Information
    Documents all animals residing at the property with breed details, health/vaccination status, and whether each qualifies as an assistance animal.
  6. Emergency Contact
    Records a single emergency contact person (not a tenant or occupant) with full address and contact details.
  7. Privacy Policy
    Notifies tenants that a privacy policy is available from the property manager or landlord's agent upon request.
  8. Tenant Signatures
    Tenant-only signature block with space for up to four tenants and dates.
