    "AMENDMENT TO THE COMMERCIAL LEASE BETWEEN THE UNDERSIGNED PARTIES CONCERNING THE LEASED PREMISES AT ___" and "Effective on ___, Landlord and Tenant amend the above-referenced lease as follows." The premises are repeated in the running header of page 2.
  2. A. Leased Premises
    "❑ A. Leased Premises: The suite or unit number identified in Paragraph 2A(1) is: ❑ (1) changed to ___. ❑ (2) contains approximately ___ rentable square feet ("rsf")."
  3. B. Term
    "❑ B. Term: ❑ (1) The length of the term stated in Paragraph 3A is changed to ___ months and ___ days. ❑ (2) The Commencement Date stated in Paragraph 3A is changed to ___. ❑ (3) The Expiration Date stated in Paragraph 3A is changed to ___."
  4. C. Rent

  5. D. Security Deposit
