The TXR 2212 form is organized into the following sections:

    Identifies the applicant, landlord, date, and property address associated with the denied rental application.

    Discloses the applicant's credit score, the date it was created, the scoring range, and the key factors that negatively impacted the score. Only completed when the consumer report included a credit score.
  4. Signatures
    Signature block for the landlord or authorized property manager, with broker identification including associate name, broker name, license number, and firm name.
