The TXR 2212 form is organized into the following sections:

  1. Notice Header
    Identifies the applicant, landlord, date, and property address associated with the denied rental application.
  2. Adverse Action Description and Reasons
    States the adverse action taken, the reason (non-credit or credit-based), and if credit-based, identifies the Consumer Reporting Agency and provides required FCRA disclosures about the applicant's rights.
  3. Credit Score Disclosure
    Discloses the applicant's credit score, the date it was created, the scoring range, and the key factors that negatively impacted the score. Only completed when the consumer report included a credit score.
  4. Signatures
    Signature block for the landlord or authorized property manager, with broker identification including associate name, broker name, license number, and firm name.
